Required Qualifications:

Completion of diploma from an accredited post-secondary rehabilitation Therapy Assistant program or equivalent training may be considered.

Additional Required Qualifications:

Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. Strong professional judgment, initiative, and effective time management skills are necessary. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.

Preferred Qualifications:

Experience in a Continuing Care setting. Knowledge of Connect Care. Palliative Care experience.
